REGISTRATION TERMS AND CONDITIONS

1. Once registered, a research investigator may order materials or
services that may be subject to specific conditions (eg MTA or Service
Agreements). These will be notified at the time of ordering. All
services and materials provided are intended for the use of research
investigators.

2. Each research investigator must register individually in his/her own
name. Registrations must not be shared.

3. Registration as a 'user' may be terminated on account of misuse of
the scheme or for failure or refusal to comply with the Terms and
Conditions. We require that you do not use this facility to carry out
any unlawful acts and that you are in compliance at all times with the
UK Data Protection Acts of 1984 and 1998.

4. The information given on this form will be used to set up a database
for internal administration purposes, and may be used for advising you
of new products, services and promotions etc.
